Register Your Coaches & Players

  • Fill out a Team Registration Form (DOC)|(PDF)
  • Fill out a Team Contact Form (DOC)|(PDF)
  • Fill out an Insurance Enrollment Form (DOC)|(PDF)

  • Pay the Registration Fee

Completed forms can be brought to the next league meeting or mailed/emailed.

NOTE: 2021 forms are being updated for 2021. Prior to playing your first game:
  • Fill out a Team Roster
  • Submit a printed Team Roster along with a completed Player Participation Form and copy of a birth certificate for each player on the team
  • Submit a completed Manager/Coach Participation Form and proof of completion of concussion awareness training as required by the State of Michigan for each coach on the team

Paperwork can be turned in at any league starting in January or paperwork drop-off meetings. Please submit paperwork for all players at one time.

2020 Season Fees

Registration includes:
  • USSSA and AABC sanctioning fees
  • Insurance
  • Two (2) dozen game balls
$550 (All Age Groups)
