Register Your Team

Register Your Coaches & Players (2025)

Completed forms can be brought to the next league meeting or mailed/emailed.

Prior to playing your first game:
  • Fill out a Team Roster
  • Submit a printed Team Roster along with a completed Player Registration Form, a Player Waiver Form and copy of a birth certificate for each player on the team
  • Submit a completed Manager/Coach Registration Form, a Manager/Coach Waiver Form and proof of completion of concussion awareness training as required by the State of Michigan for each coach on the team. Per the State of Michigan rules, concussion awareness training must have been taken within the last 3 years. For 2025, certificates must be dated on or after 9/1/2022.

Paperwork can be turned in at any league starting in January or paperwork drop-off meetings. Please submit paperwork for all players at the same time.

Registration includes:
  • USSSA team membership fee
  • Insurance
  • One (1) dozen game balls
