Prior to playing your first game:
- Fill out a Team Roster
- Submit a printed Team Roster along with a completed Player Registration Form, a Player Waiver Form and copy of a birth certificate for each player on the team
- Submit a completed Manager/Coach Registration Form, a Manager/Coach Waiver Form and proof of completion of concussion awareness training as required by the State of Michigan for each coach on the team. Per the State of Michigan rules, concussion awareness training must have been taken within the last 3 years. For 2024, certificates must be dated on or after 9/1/2021.
Review the Team Paperwork Packet Guidelines for additional details on paperwork that needs to be submitted.
Please submit paperwork for all players at the same time.
