Register Your Team

Register Your Coaches & Players (2027)

• Fill out a Team Registration Form
• Fill out a Team Contact Form
• Fill out an Insurance Enrollment Form

• Pay the Registration Fee

Completed forms can be brought to the next league meeting or mailed/emailed.

• Fill out a Team Roster
• Submit a printed Team Roster along with a completed Player Registration/Waiver Form (2027 version under review) and copy of a birth certificate for each player on the team
• Submit a completed Manager/Coach Registration/Waiver Form (2027 version under review) and proof of completion of concussion awareness training as required by the State of Michigan for each coach on the team. Per the State of Michigan rules, concussion awareness training must have been taken within the last 3 years. For 2027, certificates must be dated on or after 9/1/2024.

Coach and player paperwork can be turned in at any league starting in January or paperwork drop-off meetings. Please submit paperwork for all players at the same time.
